The Oakland Community Link Service provides support to people over 65 years old or have dementia, to access services in their local community.
A trained and experienced support team can offer short term support to older people and people with dementia, who live independently in the community.
Examples of the type of support are:
• accessing community facilities or activities
• independent travel
• meeting new friends
• building confidence
• increasing well-being
The short term service is free of charge. However there may be costs if a person requires ongoing support to use some other services or facilities.
